Telephone support for those with long term health conditions in Hull and East Riding 

Can you help people with long-term health conditions in Hull & East Riding?

Hull CVS Health Captains are looking for volunteers for our Long-term Health Conditions Buddy Service, to help others through their care journey. 

If you have experience of living with a long-term health condition or have cared or supported others with the same, we would like to hear from you. 

The role will involve offering a friendly ear to provide practical support and advice over the telephone to those who are living with a long-term health condition. 

Days and times are flexible and can work around you and there is also options to volunteer from home. 

Tasks and Responsibilities

• Calling on the phone and checking on the clients wellbeing.
• Offering your lived experience to provide peer support to others.
• Escalating any healthcare/ social care needs when needed.
• Clear and accurate record keeping and submitting of case notes to project lead.
• Signposting clients to appropriate and helpful information, services and support groups.
